Community Hearing Aid Program, Inc. Eligibility Restrictions

Restrictions: Applicants must meet residency requirements, financial and audiological criteria and have either a medical clearance or signed medical waiver. The applicant must be a legal resident of Arizona and must provide documentation to that effect. The patient’s income must be below 150% of the Federal Poverty Level. The is modified annually and can be found at the United States Department of Health and Human Services web site. The minimum hearing loss for consideration is as follows: The auditory threshold of any of the frequencies 500, 1000, 2000, 3000 or 4000 Hz must be 40 dBHL or greater and the thresholds for at least 3 of these frequencies must be 26 dBHL or greater. This is the minimum hearing loss to be considered, however, this loss does not guarantee approval in the program. CHAP, Inc. is providing binaural fittings and if an applicant elects to receive only one hearing aid, they must sign the binaural waiver.

  • A new program is being developed and CHAP is currently looking for financial support to provide hearing aids to those who fall between 151% - 250% and have been denied services from Vocational Rehabilitation. Details of this program will be released after funding has been received and eligibility criteria have been finalized.

Approval Process: All applications need to be accompanied by a complete audiological examination, a signed medical clearance or signed medical waiver, signed binaural waiver if a single aid is being requested and a $75/hearing aid application fee. Additionally, proof of residency and income is required to receive services from CHAP. Please see the application for acceptable documentation. All documents should be copies, NOT originals (these are not returned). Complete applications are reviewed as they are received.
