How to apply for services

Fitting: After the applicant has been approved, the provider and the applicant will receive approval paperwork by mail and or by fax. The paperwork will include an approval letter, audiogram, medical clearance or waiver, recipient agreement, BTE / ITE list, hearing aid request form and recipient agreement. All CHAP Network Preferred Providers donate their time and expertise and are not paid for their services.

Once the approval paperwork is received by the CHAP Network Preferred Provider, the applicant will be contacted by the professional and an appointment to have a consultation and impression of the ear will be made. The approval paperwork sent to the provider will include specific information about the approval, including the number of aids to be provided and the account numbers to be used for earmold and new In-The-Ear hearing aid orders. After meeting with the patient and taking the ear impressions, the provider will review the list of aids available and select the most appropriate hearing aid for the patient’s hearing loss and their listening demands.

Hearing Aids Available: CHAP has very limited resources and we try to make our dollars go as far as possible while still providing appropriate amplification to all our applicants. It is to that end that any new hearing aids CHAP purchases will be digital technology. CHAP only orders hearing aids from those manufacturers that provide extremely discounted prices; therefore there are a limited number of companies that CHAP works with. We have worked hard on price negotiations to be able to provide a good representation of the technology in the marketplace today. As well, the only donated BTE hearing aids that are refurbished are those under 3 years old and of mid level digital technology or higher. All other donated hearing aids are sent to an all make facility for salvage credit on repairs and new orders.

Repairs: Please be aware that CHAP pays for ALL repairs needed on hearing aids. If a CHAP, Inc. hearing aid needs to be repaired, the CHAP Network Preferred Provider who fit the hearing aid will contact CHAP and together they will determine the necessary repairs and pay for those services.

Batteries and Maintenance: It is the hearing aid wearer’s responsibility to pay for the batteries and routine maintenance of the hearing aids as well as re-tubing earmolds. CHAP will provide one new earmold every other year to qualified applicants, a $35 application fee / earmold is required. To receive a new earmold, the earmold request application needs to be completed.

No Show Policy: CHAP is very conscious of the hearing professional’s time. It is the policy of CHAP, Inc. that after one missed appointment, the patient can schedule another appointment. If a second appointment is missed, the file will be closed and no further assistance will be provided. We rely on the support of the hearing professionals in our network and we will do all that we can to be respectful of their time.
